Coaching is an ongoing relationship between a coach and a person/people who desires coaching. I agree that:

  1. Coaching is not therapy, counseling, advice giving, mental healthcare, or treatment for substance abuse or any addictive behavior. The coach is not functioning as a licensed mental health professional, therapist or counselor, and coaching is not intended as a replacement for counseling, psychiatric interventions, treatment for mental illness, recovery from past trauma, professional medical advice, financial assistance, legal counsel, or other professional services.
  2. Coaching is for people who are basically well-adjusted, emotionally healthy, functioning fairly effectively and wanting to make improvement in their lives.
  3. Coaching is designed to address issues the client would like to consider. These could include (but are not limited to) personal foundation, career development, relationship enhancement, spiritual growth, healthy lifestyle management, life balance, decision-making, and achieving short-term or long-term goals.
  4. Group Coaching is a partnership that is designed to last for the duration of the group sessions but may take longer for complete success. I understand that coaching requires completing weekly goals, work that may be challenging, and answering questions that may be difficult.
  5. Coaching can involve brainstorming, values clarification, completion of written assignments, education, goal-setting, identifying plans of action, accountability, making requests, agreements to change behavior, examining lifestyles, and responding to questions from the coach.
  6. Coaching is most effective when all parties are honest in their communication.
  7. Coaching involves payment for services. I agree to pay the fees for the duration of ADULT or YOUTH - (please select below) Group Coaching services as listed today on the FranVanCoaching.com website. I understand that group sessions occur at preset times. I understand that the coach may not be able to reschedule a previously scheduled group session to accommodate conflicts or refund payments for missed sessions. I understand that I must cancel or reschedule individual coaching calls at least 24 hours in advance to reschedule.
  8. Coaching is a confidential relationship, and the coach agrees to keep all information strictly confidential, except in those situations where such confidentiality would violate the law. I will not share what I have learned about group members with other people outside of the coaching group.
  9. Coaching assumes that each person in the relationship is guided by his or her values and beliefs. Your coach is a follower of Jesus Christ and seeks to live in accordance with this commitment. Your coach is honest in making this revelation and respects the different values and beliefs of others. Your coach does not seek to impose her values on another, proselytize, condemn, or refuse coaching services to people who do not share similar values and beliefs.
  10. I am committed to creating an alliance with my coach, Frances Van Voorhis, PhD, CPLC, and the members of my coaching group.
  11. Group Coaching is a partnership that entails commitments among the group members to offer emotional support, collective wisdom, and mutual accountability. I will strive to be open and vulnerable during group sessions to encourage connection and increase individual growth. I will listen and affirm other group members when personal issues surface. I will refrain from giving unsolicited advice to other group members. I will not monopolize the group session time because I recognize that optimal sessions require input from all group members.
  12. Accountability is critical to the success of coaching. I promise to complete all required paperwork at least 24 hours prior to our first session. I also agree to complete all assignments to maximize the benefits of group coaching.

Each person whose signature appears below agrees that this agreement represents our mutual understanding of the coaching relationship.

DISCLAIMER: The client is the sole decision-maker in the coaching process. Any and all actions or consequences resulting from coaching sessions are the responsibility of the client. The client releases the coach of all liability pertaining to services rendered in the coaching relationship.
